REQUIRED DOCUMENTATION. To receive ANY cash prize, award, or winnings, each individual winner (or the designated recipient for a team) must first provide: (a) a completed and signed IRS Form W-9 (Request for Taxpayer Identification Number and Certification); and (b) a clear photocopy or image of a valid, unexpired government-issued photo identification matching the W-9. 2. NO PAYMENT WITHOUT DOCUMENTATION. Winnings WILL NOT be paid, released, or distributed until the required W-9 and photo ID are received and verified. If a winner fails or refuses to provide this documentation within the time we specify, the winner forfeits the prize, and we may, at our discretion, award it to another eligible participant or withhold it. 3. TAX REPORTING. Cash prizes and winnings may be reported to the IRS (for example, on Form 1099) as required by law. I am solely responsible for any taxes owed on winnings. We may withhold amounts where required by law. 4. ELIGIBILITY. Winners must be at least 18 years old (or the prize is paid to the parent/guardian of a minor on the minor's behalf with the same documentation), must provide accurate taxpayer information, and must be otherwise eligible under the program rules. 5. ACCURACY. I certify that all tax and identity information I provide is true and correct, and I understand that providing false information may disqualify me and may be unlawful.
